Telehealth Consent

By accepting this consent and using Lifespan Logic to receive care, you confirm you understand and agree to the following.
1. What telehealth is
Telehealth means delivering healthcare services using telecommunications — web forms, secure messaging, audio, video, and asynchronous review of your records. Through Lifespan Logic, licensed clinicians review your intake and any uploaded records and (where clinically appropriate) prescribe medications and direct follow-up care.
2. Benefits
- Convenient access to a licensed clinician without traveling.
- Faster turnaround than many in-person clinics.
- A unified record of your visits, labs, biomarkers, and prescriptions.
3. Risks and limits
- The clinician cannot perform a physical exam or directly observe you in person, which can reduce the information available for some decisions.
- Some conditions are outside the scope of telehealth and require in-person care; if so, the clinician will tell you and may decline to prescribe.
- Technical issues (lost connection, lost data) can occur. Your clinician may need to follow up with you separately if that happens.
- Lab results uploaded by you cannot be independently verified unless they come directly from the laboratory.
4. What telehealth is NOT
Telehealth through Lifespan Logic is not for emergencies. If you are experiencing chest pain, suicidal thoughts, severe bleeding, signs of stroke, or any medical emergency, call 911 or go to the nearest emergency department. Lifespan Logic clinicians do not provide urgent or emergency care.
5. Your responsibilities
- Provide accurate, current, and complete information.
- Disclose all medications, allergies, and relevant medical history.
- Use the medications prescribed only as directed.
- Tell us promptly about side effects or worsening symptoms.
- Keep your account credentials private.
6. Privacy and security
Your records are protected under our Notice of Privacy Practices, HIPAA, and state privacy laws. PHI is encrypted at rest, transmitted over TLS, and shared only with clinicians, pharmacies, and other parties necessary to provide your care, each under a Business Associate Agreement where applicable.
7. Prescriptions and pharmacy
A prescription is not guaranteed; the decision is the clinician’s. Where prescribed, medications are filled by independent licensed pharmacies and shipped to your address. Refills may require additional information or follow-up visits, which the clinician will determine based on the protocol and applicable state rules.
8. State-specific terms
Some states impose additional rules on telehealth (synchronous-visit requirements, ID verification, age limits, advertising restrictions). Where they apply, those rules become part of this consent. Lifespan Logic will not provide care in states where the protocol you select is not authorized.
9. Right to revoke consent
You may revoke this consent at any time at /account/privacy/revoke-consent. Revocation does not affect care already provided but will end future telehealth care through Lifespan Logic.
10. Acknowledgement
By accepting this consent during sign-up, you confirm that you have read and understood the above, that you have had the opportunity to ask questions, and that you consent to receive telehealth care from Lifespan Logic-affiliated clinicians.
