Terms of Service

These Terms of Service (“Terms”) govern your use of the Lifespan Logic website at lifespanlogic.com, the patient app at app.lifespanlogic.com, and any related services (collectively, the “Service”) offered by Lifespan Logic, Inc. (“Lifespan Logic,” “we,” or “us”). By creating an account or using the Service, you agree to these Terms and to our Privacy Policy and Notice of Privacy Practices.

1. The Service

Lifespan Logic is a technology platform that connects patients with independent licensed clinicians, third-party laboratories, and third-party pharmacies. Lifespan Logic itself does not practice medicine, dispense prescriptions, or operate a pharmacy. Clinical decisions are made by your treating clinician based on your intake, lab results, and any synchronous interaction. Prescriptions (where written) are filled by independent pharmacies licensed in your state.

2. Eligibility

• You must be 18 years of age or older.
• You must reside in a U.S. state where the protocol you select is available.
• You must provide accurate, current, and complete information during sign-up and intake.
• You may not use the Service on behalf of someone else without their explicit authorization.

3. Not for emergencies

The Service is not for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency department. Lifespan Logic clinicians do not provide urgent or emergency care.

4. Account responsibilities

• You are responsible for keeping your account credentials confidential.
• You agree to notify us promptly of any unauthorized use of your account.
• One person, one account. Sharing accounts is prohibited.

5. Payment, billing, refunds

• You agree to the prices presented at checkout. Subscription services renew automatically until cancelled; you may cancel at any time at /account.
• Visit fees are non-refundable once a clinician has reviewed your intake. Medication shipment fees are non-refundable once shipped.
• If a clinician declines to prescribe, the visit fee for that visit is refunded.
• Payment is processed by our third-party processor (Stripe). We do not store full card numbers.

6. Prohibited uses

You will not:

• Provide false health information or impersonate another person.
• Use the Service to obtain controlled substances outside the scope of legitimate medical care.
• Resell, redistribute, or transfer prescriptions you receive.
• Reverse-engineer, scrape, or interfere with the Service.
• Harass clinicians or staff.

We may suspend or terminate accounts that violate these Terms. We may also report violations to law enforcement and licensing boards as required.

7. Intellectual property

The Service, its content (excluding your personal data), and the Lifespan Logic name and marks are owned by Lifespan Logic and its licensors. You receive a limited, non-transferable, revocable license to use the Service for your personal, non-commercial healthcare needs.

8. Third-party services

The Service relies on third-party providers (clinicians, pharmacies, laboratories, identity verification, payment processing, e-prescribing, messaging, and cloud hosting). Lifespan Logic is not responsible for the independent acts or omissions of these providers, but we choose them carefully and require contractual safeguards.

9. Disclaimers

THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTY OF ANY KIND. LIFESPAN LOGIC DOES NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF HARMFUL COMPONENTS. THE SERVICE IS NOT A SUBSTITUTE FOR YOUR PRIMARY CARE PHYSICIAN OR FOR EMERGENCY MEDICAL CARE.

10. Limitation of liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, LIFESPAN LOGIC’S TOTAL LIABILITY FOR ANY CLAIM ARISING OUT OF OR RELATED TO THE SERVICE WILL NOT EXCEED THE AMOUNT YOU PAID LIFESPAN LOGIC IN THE 12 MONTHS BEFORE THE EVENT GIVING RISE TO THE CLAIM. WE WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, OR PUNITIVE DAMAGES. THIS DOES NOT LIMIT LIABILITY THAT CANNOT BE LIMITED BY LAW (FOR EXAMPLE, GROSS NEGLIGENCE OR INTENTIONAL MISCONDUCT).

11. Dispute resolution; arbitration

You and Lifespan Logic agree to resolve any dispute arising out of or related to these Terms or the Service through individual binding arbitration administered by the American Arbitration Association (AAA) under its Consumer Arbitration Rules. You waive the right to a jury trial and to participate in any class action. You may opt out of arbitration by emailing legal@lifespanlogic.com within 30 days of first accepting these Terms.

12. Governing law

These Terms are governed by the laws of the State of Delaware, without regard to its conflict-of-laws rules.

13. Changes

We may modify these Terms. Material changes will be posted on this page with a new effective date and, where required, notified by email at least 30 days in advance.
